National Health and Fitness Day Employer Evaluation Form


  • Participants must engage in 30 minutes of physical activity. The 30 minutes of exercise does not need to be consecutive.
  • Participating organizations must track the number of employees that participate in the challenge.
  • Participating organizations must submit a brief evaluation after completion of the event.
  • Organizations will be divided into these categories:

*Paid participation is defined as employees who are compensated for the entire 30 minutes of exercise.


  • Manitowoc and Two Rivers community walk along the lakeshore with stretching at midpoint (5:30 p.m. start at either Lighthouse Inn or the YMCA. You can be shuttled back to your start point.
  • Defiant Fitness, Holy Family Wellness Center & YMCA will be FREE of charge for all non-members on this day, Click Here for class schedules.
  • Instructors are available to visit your work place.  Call to set-up a time!

  • Sidekick Studio will be holding a FREE Tae Bo class in the Manitowoc Yacht Club parking lot at 6:30pm (weather permitting).
  • Lakeshore Tae Kwon Do will be holding a FREE Martial Arts Fitness Class (open to all levels of experience) from 7:00 to 7:45pm at their location 823B Jay St., Manitowoc.

  • Participating organizations that turn their evaluations form in by the deadline will be entered into a drawing for a chance to win $50 in Chamber Bucks! Two winners will be drawn.
